Medicare plan availability disclosure

Medicare Advantage (Part C)

These plans provide your Part A and Part B benefits through a private insurer. Most include Part D prescription coverage.

A review can consider doctors, hospitals, premiums, cost sharing, and plan rules.

Prescription Drug Plans (Part D)

Standalone Part D plans provide prescription coverage separately from medical coverage. Review your exact medications, pharmacy, formulary, and costs.

Before changing a drug plan, confirm how the change would affect your existing coverage.
